Call the front desk during clinic hours, or leave your name, phone number and a time window on the request form. Somebody calls you back. There is no chat bot, and no form here asks you to describe a symptom.
Because medical information does not belong in a web form. Results, prescription renewals and anything else clinical are handled by phone with the clinical team or in the exam room.
Ask the desk for the direct-pay price before anything is scheduled. The prices we publish for common visits are on the insurance page.
We ask for at least 24 hours notice if you need to reschedule, so the time can go to another patient who is waiting.
