Clinic hours
Monday–Thursday 7:30 AM – 5:00 PM · Friday 8:00 AM – 3:00 PM
After hours
Outside clinic hours, call the front desk number and follow the after-hours instructions. For a life-threatening emergency, call 911 or go to the nearest emergency department.
Before you arrive
What to bring
Have these ready at the desk so the visit starts on time and the claim goes out correctly. Bring paper forms with you rather than uploading anything here.

Driver's license, state ID card, or passport for identity verification.
Current Insurance & Dental Cards
Physical card or clear digital image of front and back showing group and member IDs.
Prescription & Supplement List
Exact names and dosages of all current prescription medications, OTC remedies, and vitamins.
Relevant Prior Records or Immunization Cards
Bring physical paper copies of past vaccine records or recent specialist consultation notes.
